Clinical Pharmacist

Key Responsibilities

• Provide clinical pharmacy services to optimize medication therapy and patient outcomes.
• Review and validate medication orders, ensuring safety, appropriateness and guideline-based use.
• Collaborate with multidisciplinary healthcare teams in patient care and ward rounds.
• Monitor and report adverse drug reactions and medication-related issues.
• Provide patient and caregiver counselling on medication use.
• Participate in formulary management, medication audits and quality improvement initiatives.

Qualifications & Experience

• A Bachelor of Pharmacy (B.Pharm) degree from a recognized university.
• Registration with the Sri Lanka Medical Council as a Pharmacist.
• Relevant experience in a hospital or clinical pharmacy setting will be an added advantage.
• Strong clinical knowledge, communication and interpersonal skills.
• Commitment to patient safety and continuous professional development.
